1. Pulmonary cyst/bullae & blebs

• 폐 쪽에 생기는 낭. Cyst/bullae/blebs로 나눈다.
• 가장 무서운 경우는 rupture. 갑자기 호흡곤란이 발생할 수 있음.
• 확진은 조직검사로만 가능
• Cyst: 상피로 전체가 둘러싸여 있는 것.(실질에 생긴 문제)
• Bullae: internal & external 둘 다 약해짐.
• Bleb: external이 늘어나 공기가 참. → 좀 더 쉽게 관찰 가능.
• 한번 발생하면 고치기 어려움.

What’s your diagnosis?

Bulla	Bleb
실질과 비슷하게 보임. 깊은 쪽에서 발생 (안쪽에서도 문제가 되기 때문) 만지면 서걱서걱.

• Thoracocentesis: 흉강에 있는 공기나 액체를 제거하는 방법으로, 길어봐야 3-4일 효과가 지속됨.
• 실제로 개흉 → 구멍 봉합.
• Partial lobectomy: 손상된 폐엽을 부분적으로 절제하는 방법.

2. Pulmonary Abscess

• Uncommon
• 배수를 잘 못해서 생기는 경우가 많음.
• Secondary to
  • Foreign body obstruction (배출로가 차단됨 → 정체 → 감염 → 농양)
  • Foreign body migration (다른 곳에서 이물이 뚫고 넘어오면서)
  • Chronic lung infection
  • Neoplastic disease (역시 막히는 종류)
• 항생제를 4주 이상 -6주까지 써보는 것이 원칙.
  • But 동물에서 약으로 잘 잡히진 않음. 초반에 오는 경우가 잘 없기 때문.
  • 흉강을 열고 농양 부위를 lobectomy 하는 경우가 대부분.

3. Restrictive pleuritis

• Heavy fibrin deposition
• Surgical treatment :
  • 1. Lobectomy
  • 2. Lung decortication : 박피 (뜯어내는 것)

4. Lung laceration

• Cause
  • Iatrogenic : Thoracocentesis, thoracostomy tube, inadvertent cut during thoracotomy
  • Traumatic : 봉합 꼭 필요 (often fatal)
    • Rib fractures, penetrating trauma, Foreign body displacement from esophagus

Respiratory distress

• 호흡곤란 발생 시, : 반드시 순차적으로 조치해야 함.
• Treatment - conservative
  • 1. Oxygen supplement : 산소 공급
  • 2. Thoracocentesis : 공기를 빼 줌
  • 3. Thoracostomy tube
• Treatment - surgical
  • 웬만큼 해도 기흉 잡히지 않을 때 (uncontrollable)
  • 흉강내 출혈이 >2ml/kg/hr 로 지속될 때
  • Mild한 출혈 - inverting suture pattern(ex. Lembert)
  • Severe한 출혈 - horizontal mattress pattern + everting된 부분에 simple continuous closure
  • 그래도 안 되면 partial or complete lobectomy
  • 요즘은 tissue glow로 마무리하기도 [Tissumend Ⅱ]

5. Lung lobe torsion

• Uncommon
• Rt. middle lobe에서 가장 common : Rt. cranial, Lt. cranial에 따라옴
• Radiography : congested, consolidated lobe
• 정체되면서 흉수 발생 가능
  • 유미(chylous)가 흉강에 모임
• Treatment : lobectomy - 풀어버리면(untwist) 고여 있던 모든 것들이 온몸으로 퍼지며 사망 가능.

Thoracic duct (🔗 김내2(A) - 3.9 Intestinal lymphangiectasia (림프관확장증))

• 유미를 이동시키는 thoracic duct는 extrajugular vein에 termination
  • 혹은 subclavian jugular.. 그 언저리, 통칭해서 Left extrajugular vein
• 이게 정체되면 chyle도 흉수 쪽으로 빠져나감

6. Pulmonary neoplasia

• 전이된 종양이 훨씬 많음 (Metastatic > Primary)
• adenocarcinoma - 끝자락에서 발생한 종양은 수술하기 정말 수월 / hilus에 발생하면 수술 어려움
• 전이된 종양은 수술적으로 접근하지 않음, 수술 외적인 것(방사선 치료 등)으로 치료
• 외과적 적용도 계속 해봤더니.. 3개 이하의 osteosacoma에 한해 수술적 제거로 수명 연장 가능하다는 결과
• Treatment : Lung lobectomy (complete or partial)
  • → complete = 호흡이 조금 어려워져도 완전 제거
  • 폐의 50%가 없어져도 살 수 있음
  • partial로 재발의 위험을 남겨두는 것보다..

What’s your diagnosis?

• 3cm 이상이면 전이될 가능성 크다고 봄
• Left caudal margin에 adenocarcinoma

Partial Lung Lobectomy with Staples

• Excisional biopsy : 다 들어냄
• Hemostatic foscep(crush)로 결찰
• Margin은 가질 수 있을 만큼 많이 확보
• 봉합 시 기흉과 출혈 반드시 막아야 : 봉합과 봉합 사이가 겹쳐야 한다 (overlapping suture)
  • 봉합했던 부분을 다시 스쳐 지나가며 겹쳐서 봉합. 마무리할 때 직전 봉합을 겹쳐..
  • 교수님은 이중으로 한번 더
  • 단면이 불안하면 simple contrinuous로 margin 봉합 (= oversawing)
  • 작게..
  • Stapler 박아서 마무리 (지그재그로 - 물샐 틈 없이)

Complete

• 결찰 순서 : 동맥 - 정맥 - brunchus (simple interrupted)

• Horizontal mattress - continous 하게.

• 

Pneumonectomy

• 오른쪽 or 왼쪽 폐 전체를 들어내는 것.
• 오른쪽 폐가 55-60%를 차지 : 현실적으로 왼쪽 폐에만 적용 가능함.
