임병1(A) - 2-2. Lymph nodes (Ch.6)

• cortex(=lymphoid follicle)과 medulla로 구분
  • germinal center, cap의 위치에 따라 분포하는 림프구의 종류가 다름.
• LGB (lymphoglandular body) : 림프절의 cellularity가 높아 손상된 세포들의 세포질 조각,림프구가 아주 많은 장기의FNAB에서 관찰되는 소견.

Normal lymph node

• mature lymphocyte가 관찰됨. Lymphoblatic, lymphocyte도 함께 발견됨.

Plasma cell inclusion

• Mott cell : 염증
  • Russel body : IgG가 inclusion처럼 관찰되는 것. refractile blue green cytoplasmic inclusion
  • inflammation
• Dutcher body : 염증 & 종양
  • intranuclear inclusion
  • plasmacytoid lymphoma

• a : 작은 크기의 Russell body들이 잔뜩 뭉쳐 있음.

• b : 정상 plasma cell과는 다른 N:C ratio

• c : 핵과 세포질의 염색성이 낮고, chromatin pattern이 일정하지 않음.

• d : 핵의 chromatin의 응축 소견

• e : 그나마 정상 세포에 해당.

• LGB (Lymphoglandular body) : 림프절의 cellularity가 높아서 발생하나 손상된 세포의 세포질 조각. 연한 파란색으로 관찰됨. 림프구가 아주 많은 장기의 FNAB에서 관찰됨.

• 정상적으로 림프절에서는 mature lymphocyte와 lymphoblast가 함께 관찰됨.

  • 개에서 RBC와 mature small lymphocyte의 크기는 비슷.
  • 고양이에서는 RBC가 더 작다.

• Russel body : IgG가 inclusion처럼 관찰됨. refractile blue green cytoplasmic inclusion

• 염증 소견 : 정상적으로는 소수만 존재해야 하는 neutrophil과 plasma cell의 숫자가 많이 늘어나 있다.

• 정상적인 plasma cell

  • N:C ratio가 1:2, perinuclear halo, 핵의 위치가 eccentric하지 않음.

• malignent를 나타내는 비전형 특징들

  • 세포질의 tailing
  • serration ; 세포질의 돌기
  • indentation : 핵이 함입되어 있다.
  • 핵의 pleomorphism
  • 불균질한 chromatin pattern
  • N:C ratio variation
  • binucleated cell

• 대부분 세포질을 가지고 있어 lymphoblast로 판단한다. → lymphoblast type의 malignant lymphoma

• MMC(Macronucleated medium cell) : medium-size의 세포로, 가운데에 큰 핵소체가 하나 존재한다. 핵의 크기가 RBC, small lymphocyte와 비슷하다. 개에서만 관찰된다.
