Rabies

• acute, fatal zoonotic disease
• 사람의 공수병(hydrophobia), 동물의 광견병
• Etiology : rabies virus (genus Lyssavirus의 serotype 1)
  • 거의 serotype 1에서 발생하나, serotype 2~7에서도 발생 가능한듯
• Occurence
  • urban type (개발도상국developing) : 유기견, 박쥐 → 반려동물, 가축 → 사람
  • sylvatic type (북반구 선진국developed) : 야생동물 → 반려동물, 가축 → 사람
  • 제3세계(Third World countries)에서 most common
  • 우리나라 : 사람은 거의 없으나, 가축은 많이 감염되고 있음.
• Transmission
  • saliva by the bite of infected animal (감염된 동물에 의한 교상, 타액, 점막)
  • close contact with infected (w/o bite), aerosols in bats (bat→인간 전파는 없는듯(족보))
  • cannot penetrate intact skin (정상 피부에는 침투 불가)
  • peripheral nerve → CNS → salivary glands (→ 침에 많은 바이러스 포함)
• Clinical manifestation
  • 광폭형(furious)과 마비형(paralytic), fatality rate은 마비형 > 광폭형 (족보)
  • 잠복기(incubation period)는 평균 3개월 (5일 ~ 2년까지 다양)
  • the closer to CNS, the shorter I.P
  • vomiting, headaches, slight fever
  • 증세가 나타나면 반드시 death
• Therapy & Prophylaxis
  • vaccination postexposure : 교상 후에도 빠르게 백신 맞기
  • no treatment after symptoms
  • 반려동물도 vaccination
