김내2(A) - 3.12 Intestinal leiomyoma, leiomyosarcoma, stromal tumor

Etiology

• Connective tissue tumors (결합조직 종양) : Distinct mass, 경계를 가지고 focal하게 자람.
  • 조기 발견 시 수술적 절제가 가능할 수도.
• Small intestine & stomach에서 발생
• Older dogs : 나이든 개에서 많이 생김.

Clinical signs

• Intestinal hemorrhage (장출혈)
• Iron deficiency anemia (철 결핍성 빈혈)
• Obstruction (폐색)
• Hypoglycemia : paraneoplastic effect로 발생 (🔗LL의 paraneoplastic effect; PTH-rP)
  • 종양에서 IGF-2(Insulin-like Growth Factor 2) 분비 → 인슐린과 비슷해서, 혈당이 저하됨.
  • 잘 먹는데도 혈당이 떨어지면 결합조직 종양 의심.
  • Glucose 낮은데, insulin도 낮음.

Diagnosis

• FNA
• biopsy : sarcoma의 경우 FNA로 샘플 확보가 어려울 수도 있음.

Treatment &Prognosis

• Surgical excision ⇒ 완전 치료 가능
• Palliative chemotherapy : 완치는 아니지만 완화적 처치.
• 전이가 있으면 예후가 나쁨.
