  • 너무 팽팽한 곳을 그냥 풀어주는 - 절개를 하거나
• Plane of undermining : direct cutaneous vessel을 포함할 것

  • Excessive tension, rough technique, and division of direct cutaneous arteries interfere with cutaneous circulation, potentially causing skin necrosis, dehiscence, or infection

  • 상처 부위 주변 정상 조직을 느슨하게 하는 느낌
  • Simple relaxing incisions : tension이 많으면 앞쪽에 절개창: 상처있는 곳은 잘 봉합해놓고, 정상인 조직은 약간 절개되어도 괜찮음
  • Multiple Punctate relaxing incisions : 상처 주변에 구멍을 뻥뻥 뚫어 tension 완화
• Skin Stretching & Expansion
  • Presuturing : 버튼을 이용해서 처음엔 느슨하게 → 버튼 쪽에서 실을 땡겨서 조금씩 닫아주는
  • Stretchers? Expanders? : Flap 땡겨올 피부가 없을 때, ballon이나 물로 피부를 부풀려서 최대한 느슨하게 (괴사 위험 감수) ⇒ 상처 쪽으로 떙겨옴
• Suture patterns
  • Walking sutures : tension이 한쪽에 너무 몰리지 않도록 중간중간 tension을 분산할 수 있는 anchor 를 두는 것
  • External Tension-Relieving Sutures :
• Dog ears : 피부가 마지막에 우는 것
  • 처음부터 잘..
  • 한쪽 끝에서 일자로 툭툭 하다보면 울 수 있음 (특히 굴곡 있는 부위에서)
  • 중간을 먼저 봉합하고 중간 중간 나눠가면서 하면 dog ear 방지하기 좋음
  • 생기면 → 줄을 좀 더 그어주거나, 사각형으로 작게 작게

Ⅰ. Introduction

• 재건 수술의 목적: 외상, 선천적 이상 교정, 종양(neoplasm) 제거 후 결손 부위 폐쇄를 위해 흔히 시행됨.
• 상처 치유(wound healing)와 폐쇄(closure): 큰 trunk 병변은 수축(contraction)과 상피화(epithelialization)로 치유될 수 있으나, 상처 폐쇄가 더 바람직한 경우가 많음. 다양한 절개(relaxing incision), plasty(V-to-Y, Z-plasty), flap, graft 등이 있음.
• 계획 시 주요 고려사항: 상처 위치(location), 주변 조직의 탄력성(elasticity), regional blood supply, wound bed의 특성 등을 반드시 고려해야 함.

Ⅱ. Skin Tension and Elasticity

• 피부 긴장도(skin tension)와 탄력성(elasticity) 평가: flap 또는 graft 예정 부위의 피부를 잡아 들어올려 자발적 수축을 관찰함으로써 평가함. 허용 가능한 tension은 주관적임.
• 과도한 tension의 결과: 절개부 가장자리를 과도한 tension 상태로 맞대면 통증(incisional discomfort), 압박 괴사(pressure necrosis), 봉합사 절단(sutures “cutting out”), 부분 또는 완전 절개부 열개(incisional dehiscence)가 발생할 수 있음.

Ⅲ. Tension Lines Formation

• tension line의 형성: 피부 내 fibrous tissue의 주된 pull에 의해 형성됨. 품종(breed), 체형(conformation), 성별(gender), 연령(age)에 따라 다름.
• 수술적 중요성: tension은 incised skin edge를 벌어지게 하고 linear scar를 넓힘. tension line과 평행하게 incision을 하면 더 빠르고 미용적으로 우수하게 치유됨. tension line을 가로지르는 incision은 벌어지고, 더 많은 suture가 필요하며 dehiscence 위험이 높음.

Ⅳ. Tension Relief Techniques

• tension 완화 방법: wound closure 시 tension을 줄이기 위한 여러 방법이 있음.
• Undermining: 가장 간단한 tension-relieving procedure. scissors로 skin 또는 panniculus muscle(또는 둘 다)을 박리하여 피부의 탄력성을 최대한 활용함. plane of undermining은 panniculus muscle layer(존재 시) 아래로, extremity에서는 loose areolar fascia deep to dermis로 진행하여 subdermal plexus를 보존해야 함.
• Relaxing Incision: defect 인접에 skin edge apposition을 위해 시행. 주로 distal extremity, 눈(eye), 항문(anus) 주변, vital structure나 implant 덮개에 사용. 단일 relaxing incision은 25~30일 내 contraction과 epithelialization으로 치유됨. multiple punctate relaxing incision은 미용적으로 더 우수하나, 순환 장애 위험이 높음.
• Skin Stretching and Expansion: mechanical creep, stress relaxation을 이용해 skin을 본래 elasticity 이상으로 늘림. presuturing, adjustable suture, skin stretcher, tissue expander 등 다양한 방법이 있음.

Ⅴ. Suture Patterns for Tension Relief

• Subdermal Suture: subdermal 또는 subcuticular tissue에 봉합하여 skin suture에 가해지는 tension을 줄이고, scar를 감소시킴.
• Walking Suture: skin을 defect로 이동시키고, dead space를 없애며, tension을 분산시킴. 과도한 봉합은 circulation compromise를 유발할 수 있으므로 최소화해야 함.
• External Tension-Relieving Suture: suture가 skin을 절단하는 것을 방지하기 위해 pressure를 넓게 분산시킴. mattress, cruciate, pulley suture(“far-near-near-far”, “far-far-near-near”) 등 다양한 패턴이 있음.

Ⅵ. Prevention and Correction of “Dog Ears”

• “Dog ear”(봉합선 끝의 puckering) 예방: suture spacing 조절(볼록면은 촘촘히, 오목면은 넓게), 얇고 elastic한 skin에서 덜 발생함. 대부분은 excision 없이 평평해짐.
• 교정: elliptic incision으로 redundant skin을 제거하거나, triangle excision 등 다양한 방법으로 correction함.

Ⅶ. Removal of Skin Tumors

• 계획: skin tension과 elasticity 평가, excessive tumor manipulation 피함. tension line 방향, excision shape, closure method를 미리 계획해야 함.
• excision margin: tumor, previous biopsy site, 정상 조직의 충분한 여유를 포함하여 3차원적으로 절제.
• Radical Tumor Excision: poorly localized/high-grade malignant tumor에 적용. compartment structure, amputation, lobectomy 등 포함.
• blood supply ligation, wound bed irrigation, lymph node biopsy, contaminated instrument 교체 등 수술 전후 관리가 중요함.

Ⅷ. Closure of Irregular Skin Defects

• circular defect: “dog ear” 발생이 쉬워 linear, combined V, bow tie technique이 선호됨.
• linear closure: 작은 defect에 적용, suture는 tension line과 평행하게.
• combined V, bow tie technique: skin이 부족하거나 여유가 많을 때 각각 적용.

Ⅸ. Closure of Triangular, Square, Rectangular Defects

• triangular defect: Y-shaped suture line, rotational flap 등으로 closure.
• square/rectangular defect: centripetal, unilateral(single-pedicle) advancement flap, rotational flap 등으로 closure.

Ⅹ. Skin Flaps

• Pedicle Flap: 혈관이 포함된 epidermis와 dermis 일부를 donor site에서 defect로 이동. 즉시 coverage 제공, secondary intention의 scarring과 contracture 예방.
• Subdermal Plexus Flap: circulation이 subdermal plexus에서 옴. 가장 흔함.
• Axial Pattern Flap: direct cutaneous artery/vein이 base에 포함되어 perfusion이 더 좋음.
• Island flap, local flap, interpolation flap, distant flap, compound/composite flap 등 다양한 분류가 있음.
• flap fixation, donor site management, cosmetic planning, adjunct treatment(HBOT, NPWT) 등도 중요.

Ⅺ. Types of Skin Flaps (Detailed)

• advancement flap, rotational flap, transposition flap, Z-plasty, interpolation flap, pouch/hinge flap, tubed pedicle flap, axial pattern flap 등 각 기법의 적응증, 장단점, 합병증 등.

Ⅻ. Composite and Muscle Flaps

• composite flap: skin과 muscle, bone, cartilage 등 포함.
• muscle flap: herniorrhaphy, soft tissue defect coverage, infection control 등에서 사용. vascular supply가 중요.
• myocutaneous flap: muscle과 overlying skin을 함께 이동, direct cutaneous artery가 skin에 혈류 공급.

ⅩⅢ. Omental Flaps

• 목적: soft tissue defect coverage, circulation/drainage 제공, healing 촉진, adhesion control, infection control 등.
• anatomy, mobilization method, complication 등.

ⅩⅣ. Microvascular Flap Transfer

• microvascular flap transfer: specialized training, operating microscope, fine suture material(9-0~11-0), vascular clamp 등 필요. vessel anastomosis가 핵심.

ⅩⅤ. Skin Grafts

• 정의: free dermis/epidermis를 distant recipient site로 transfer.
• 종류: full-thickness(진피 전체+표피), split-thickness(진피 일부+표피). feline skin은 split-thickness graft에 부적합.
• 생착 과정: plasmatic imbibition(혈장 흡수), inosculation(혈관 연결), new vessel ingrowth(신생 혈관 성장). 7~8일 내에 이루어져야 함.
• recipient bed 조건: healthy granulation tissue, infection/debris 없음, adequate vascular supply 필요.
• failure 원인: graft-bed separation, infection, movement, hematoma/seroma, improper dressing 등.

ⅩⅥ. Types of Skin Grafts (Specific Techniques)

• full-thickness skin graft: flexor surface, distal extremity 등에서 사용. healing 후 normal skin과 유사.
• mesh graft: drainage, flexibility, expansion 증가. NPWT 병행 시 graft uptake 향상.
• split-thickness skin graft: durability 낮고, hair growth 적음. stamp graft 등 다양한 변형 기법 존재.
