• Filovirus : enveloped, nonsegmented, (-) ssRNA
  • filo=실. 필라멘트처럼 생김.
• extremly pathogenic / not highly contagious (전염력 ↓ 병원성↑)

Marburg fever

• hemorrhagic fever, high fatality rate
• Etiology : Marburg virus
  • filamentous virus particle (filo=실)
• Occurence
  • 유럽 영장류와 접촉한 실험실 사람에게서 발견
• Transmission
  • direct contact, aerosols
  • sperm에서 발견된 case 있음
• Clinical manifestation
  • high fever
  • vomiting and severe diarrhea (식중독 유사 증상)
  • 유럽 fatality rate 30%
• Therapy & Prophylaxis
  • 대증치료
  • experimental vaccine 있으나 잘 사용되지 않음
  • potentially infectious monkeys, people, material 다룰 때 주의

Ebola Virus Hemorrhagic Fever

• hemarrhagic fever, extremely high lethality (감염력은 낮음)
• Etiology :
• Occurence
  • Africa Congo 지역에서 처음 발견
  • 5 species : 4개는 사람에게 질병 유발, 1개는 NHP에게(사람X)
• Transmission
  • 전부 human-to-human
  • 병원 등 접촉 감염(patient blood, accidental inoculation, reused needles, ..)
  • consuming chimpanzees as food
  • 아프리카의 burial rituals
  • fruit bat이 host로 의심
• Clinical manifestation
  • fever, headaches, diarrhea, .. (Marburg와 유사)
  • 75% hemorrhagic
  • 1976의 fatality rate : 53%, 88%, .. (매우 높음)
• Differential Diagnosis
  • hemorrhage : only seen in patients with a fatal course (fatal할 때만 출혈 소견)
  • 다른 hemorrhagic fever(yellow fever, flavivirus 등)과 감별 필요
  • 진단과 실험은 have to be perforemed in BSL-4 facilities
• Theraphy & Prophylaxis
  • 대증치료
  • fresh-blood transfusion
  • serum from recovered patients
  • ribavirin, interferon.. but not fully tested yet (임상 시험을 마친 치료제는 없음)
  • 예방 : the deceased cremated(사망자 화장하기), 접촉 주의, 죽은 원숭이 먹지 않기
